About this Survey

With this survey we hope to gather as much data as possible to get a better understanding of how Fibromyalgia affects people. We aim to improve our services for young adults with fibromyalgia/Chronic fatigue in the age group 16-30 years living in and around Newbury. We have been awarded a grant from Healthwatch West Berkshire to carry out this service development project. We are hoping to increase awareness and promote understanding as well as provide a network of resources to these patients and their carers.

Are you eligible for the study

If you belong to any of the practices above and are aged between 16 and 30 you are eligible to take the survey and be entered into the prize draw for the £100 Amazon voucher. If you are not aged between 16-30 unfortunately you are not eligible to complete the survey . If you do not belong to these practices, but are still within the age range, your survey is still valuable and can be used for the study however you may not be included in the draw.

Any personal data you enter will only be used for research purposes and will not be shared. Name and contact details are only necessary if you wish to enter the draw. We will not call or email unless you have won the prize draw.
